About Care Management

CONSULTATION and ASSESSMENT

 

In the comfort of their own home the I will obtain a general assessment of your elder's level of cognitive, physical, social, and emotional functioning and create an appropriate plan or referral. I explore aging at home versus care in a facility, hiring caregivers from a home health agency, and referrals to medical specialists for in-depth evaluation. I also serve clients as they transition from home to assisted living facilities - offering connection and continuity as they adjust to new environments, staff, and routines.

 

 

PLAN of CARE

 

Together we will create a plan of care to ensure safety, communication, happiness, and health. This would include observing where your elder requires assistance and monitoring in their daily routines - i.e. exercise, bathing, cooking, shopping, medical appointments and communication, spiritual and leisure pursuits, and home maintenance. The plan of care will be in line with individual strengths and preferences, guidelines from the client's MD, family input, and supportive staff such as caregivers and therapists.

MEDICAL MANAGEMENT

 

Advocacy! As seniors attempt to manage their ever increasing aging issues, it is critical to have a single individual who is deeply familiar with their medical/physical and behavioral/emotional issues who can navigate and liaison between family, care staff, and medical specialists. I accompany clients to their medical appointments, consult during hospitalizations, and remain on call for crisis intervention. Information is documented and communicated, and client needs and goals adjusted accordingly. 

 

 

RELATIONSHIP BUILDING

 

My love and generosity of spirit are the benchmark of my relationships with my clients. Whether you utilize me for a brief consultation, for a few months, or many years, you can be certain that your loved one is in kind, and deeply caring hands. I love what I do, and feel incredibly rewarded as I become almost like family to many of my patients. I stay close by their side as they face some of the most difficult issues as they age - helping to cope, heal, and continue living to the fullest.

Edie is a blessing! She has worked with me for four years now, and we really click because she has such a cheerful personality. I have the staff at the assisted living helping me too, but having Edie's support for my medical appointments and extra things I need help coordinating has made me feel much less worried.  You can depend on her, and she works hard for me.
