  • Manage all household activities: maintenance and safety, adaptive equipment, communication and appointments, nutrition and shopping, personal care, and more
     

  • Educate and supervise agency and private caregivers to ensure household activities are performed with the client's nature and preferences at heart
     

  • Coordinate and collaborate with family, caregivers, community services, medical and hospice services while mindful of client's level of functioning and lifestyle

Health Care

  • Address all health care needs: diet, medication systems, adaptive equipment, exercise, therapy, medical issues
     

  • Accompany and advocate for clients during medical appointments and hospitalization
     

  • Ensure accurate and timely communication among medical specialists, clients and their families, and care staff
     

  • Guidance in transitions from hospitalization to rehabilitation and home. Assistance with rehabilitation goals/exercise programs
     

  • Emotional support in relocation and transitions to assisted living, skilled nursing, hospice, and memory care
     

  • Crisis intervention for ER visits, etc..

Well-Being

  • Ensure recommendations made by client's doctor's and therapists are implemented
              

  • Assist with activities of daily living while promoting client ease and independence
     

  • Provide cognitive, fitness, and leisure activities
     

  • ​Educate families and caregivers as to diagnoses and behavioral interventions
     

  • Build nurturing and trusting relationships
